Office hours:

Monday through Friday, 8:00am-4:00pm. Please be prompt for your appointment so we can give you and our other patients the best possible care.

Insurance:

Most insurance plans are accepted including private insurances, HMO’s, personal injury and worker’s compensation. More details.

Payment Options:

Your co-pay should be paid at the time of your visit. We gladly accept VISA, MasterCard, Discover, American Express, check, or cash. There is a $25.00 non-sufficient funds fee on all returned checks.

If you are unable to keep your appointment or if you will be arriving late, we will be more than happy in assisting you with this matter. We ask that you please contact our office within 24 hours of your scheduled appointment.

Emergencies:

In case of emergencies after regular business hours please go to your nearest emergency room.
